Routine vision coverage benefits
are only available when services are performed by a National Vision
Administrators (NVA) network provider. To find in-network
providers, you may either call NVA at 1-888-478-3722 or visit NVA’s
website by clicking here: www.e-nva.com.

No. Effective April 1, 2011
and forward there is no routine vision coverage for retirees through
National Vision Administrators (NVA).
